I'm pretty sure we're intended to think 'just bad luck' but uh my actual opinion on why so many people died is 'Em's methods of solo sleep-deprived navigational support, which she inexplicably believes are safer for her spelunkers, are in fact SIGNIFICANTLY LESS SAFE and everyone would have been more or less fine if they'd had a proper rotational support team of people getting eight hours of sleep.'
